Cognitive Remediation Program in Early Stage of Treatment for Psychosis

Cognitive impairment is the prime factor of the significant disabilities in social functioning and important target of treatment. In terms of assessment of the patients’ lifestyle and cognitive deficits, cognitive remediation programs for inpatients may lead to proper goal setting. Here we present our cognitive remediation program for inpatients. Hospitalized patients with schizophrenia less than 50 years old were targeted. Their mental state, cognitive function and social function were assessed. They received feedback based on these evaluations and were motivated to set their goals. Three staffs dealt with 5-10 patients, working on five elements of cognitive function; attention, memory, executive function, fluency and social cognition. After three sessions involving paper and pencil tasks which targeted one of five elements, “Bridging Session” aiding the transfer of cognitive gains into the real world was conducted, in which participants were encouraged to use the skills learnt to achieve their real-life goals. Forty two patients (mean age 38.0±6.5 years old) joined this program since the launch in April 2014. Eleven of 37 patients who have finished this program received further rehabilitation, and three worked in supported employment. Patients in early stage of treatment find difficulties in understanding the need for rehabilitation, however they find some value in this program due to its clear aim. In this regards, many patients who attended this program received ambulatory rehabilitation. Possibly this resulted from the effect of this program which makes each patients’ goal clearer and motivates them to receive further rehabilitation.
